Registration Downloads
To save the forms to your computer, right-click on the link and select "Save target as..." or "Save link as...".

• Registration Form (97K, PDF)
• Needed to register your group for Alive in You
• Group leader fills out the registration form and mails it to Alive in You
  
• Group Participant Spreadsheet (66K, PDF)
• Once all members of your group are signed up, the group leader will complete the spreadsheet and either e-mail or mail it to Alive in You
  
• Medical Release Form (89K, PDF)
• Every participant is responsible for completing the Alive in You Medical Release form
• Bring the originals to camp with your group
• Bring a set of copies to camp with your group
• Do not mail Medical Release forms to Alive in You
  
• Release of All Claims Form (56K, PDF)
• Every participant is responsible for completing the Alive in You Release of All Claims
